Here is a list of documents that will be needed for different types of claims:

Motor Claims

To claim the damage to the windscreen of a vehicle, the documents required are:
  • Full view photo of the vehicle BEFORE repair/replacement of the windscreen (mandatory)
  • Zoomed view of the damage or breakage BEFORE repair/replacement of the windscreen with vehicle pillars (mandatory)
  • Full view photo AFTER repair/replacement of windscreen & Vehicle's Plate No. with date imprinted (mandatory)
  • Zoomed view of the damage or breakage AFTER repair/replacement of the windscreen with vehicle pillars (mandatory)
  • Original Receipt/ Bill/Invoice with company's letter head (mandatory)
  • Photo of Chassis Number with date imprinted. (mandatory)
  • A copy of Police Report - if caused by malicious damage (mandatory)
  • Payment Instruction order/Authorization letter - if pay to non panel workshop (provide workshop's Maybank Account No.) (mandatory)
To claim for stolen vehicle, the documents required are:
  • Certified Copy of Police Report (mandatory) (mandatory)
  • Copy of Vehicle Registration Card (mandatory)
  • Copy of Insured's MyKad/ ID or Business Reg. License (if insured is a company) (mandatory)
  • Copy of Insured's Driving License (mandatory)
  • Copy of Driver's MyKad/ ID (mandatory)
  • Copy of Driver's Driving License (mandatory)
  • Vehicle Permit (applicable for commercial vehicle) (mandatory)
  • Goods Driver's License/ Public Service Vehicle License (applicable for commercial vehicle only) (mandatory)
  • Memorandum of Association (M&A) (applicable for Company's vehicle only) (optional)
  • Custom Form/Approved Permit (AP Permit) (applicable for imported car) (mandatory)

Non Motor Claims

Personal Accident (PA)

To claim for losses involving accidental injury / death to policy holders, the documents required are:
  • A copy of Police Report (applicable for Motor accident) (mandatory)
  • Medical Report (mandatory)
  • Original Medical Bills & Receipts (mandatory)
To claim for losses involving accidental injury / death to policy holders, the documents required are:
  • Claim Form completed by policyholder & the attending Doctor (mandatory)
  • Original Medical Bills & Receipt for Medical expenses (mandatory)
  • A copy of Police Report (applicable for Motor accident) (mandatory)
  • Original Receipt for Ambulance fees (if applicable) (optional)
To claim for losses involving accidental injury / death to policy holders, the documents required are:
  • Claim Form completed by policyholder & the attending Doctor (mandatory)
  • Original Hospital Bills & Receipt (mandatory)
  • Admission/ Discharge Note (mandatory)
  • A copy of Police Report (applicable for Motor accident) (mandatory)
To claim for losses involving accidental injury / death to policy holders, the documents required are:
  • Claim Form completed by policyholder & the attending Doctor (mandatory)
  • Medical Certificate (i.e. MC Slip) - applicable for temporary total disability claim (mandatory)
  • Light Duty Certificate (optional)
To claim for losses involving accidental injury / death to policy holders, the documents required are:
  • Claim Form completed by policyholder & the attending Doctor (mandatory)
  • Death Certificate/ Burial Permit (mandatory)
  • Beneficiary's MyKad/ ID (mandatory)
  • Marriage Certificate/ Statutory Declaration of deceased's marital status (either single/ married) - Mandatory for spouse (mandatory)
  • Letter of Administration from Amanah Raya or any appointed lawyer / Surat Fara'id (optional)
  • Post Mortem Report (optional)
  • Toxicology Report (if blood test done) (optional)

Medical

To claim for medical expenses incurred under Medical Policy, the documents required are:
  • Police Report (applicable for Motor accident) (mandatory)
  • Medical Report (mandatory)
  • Original Medical Bills & Receipts (mandatory)

Travel

To claim for losses or travelling inconveniences occurred during the travelling period, the documents required are:
  • Statement for the purchase of air ticket / copy of Airline Ticket/ Boarding Pass (with flight details e.g. flight no., date, departure airport, destination, scheduled times, arrival airport, etc) (mandatory)
  • Property Irregularity Report obtained from the Airline (mandatory)
  • Evidence of expenses incurred (Sales Drafts charged for which reimbursement is claimed e.g. accommodation, meals, essential items, etc) (mandatory)

Miscellaneous

To claim for losses involving Fire/ House Owner/ House Holder. 

Documents required:

  • Original/ certified true copy of replacement Invoice/ Bill/ Quotation/ Estimate (mandatory)
  • Fire Brigade Report (optional)
  • Original/ certified true copy of purchase invoice (optional)
  • Stock Card Records (optional)
  • Original close-up photographs depicting the damaged property (optional)
To claim for losses under Maxi Home policies due to Fire/ Domestic Explosion/ Lightning/ Burst Pipe/ Windstorm/ Break-in/ Flood/ Impact damage. 

Documents required:
  • Police Report (mandatory)
  • Original/ certified true copy of replacement Invoice/ Bill/ Quotation/ Estimate (mandatory)
  • Fire Brigade Report (optional)
  • Original/ certified true copy of purchase invoice (optional)
  • Original close-up photographs depicting the damaged property (optional)
Items must be purchased by using any of the listed Maybankards and is subject to terms & conditions of the policy.

Documents required:
  • Police Report (for theft claims) (mandatory)
  • Original purchase invoice (mandatory)
  • Credit card statement (or print screen) showing the purchase transaction (mandatory)
  • Credit card slip (optional)
  • Repair invoice/ quotation (for accidental damage claims) (mandatory)
  • Photos of the damaged item (for accidental damage claims) (mandatory)
To claim for reimbursement due to product dissatisfaction with the item(s) purchased solely for personal use using any of the listed Maybankards within 90 days from the date of purchase; in which the retailer has refused to accept the return of the purchased item(s). 

The insured is required to send the item to Etiqa (i.e. in brand-new condition) & Etiqa will reimburse the claimant subject to policy terms & conditions

Documents required:
  • Original purchase invoice (mandatory)
  • Credit card statement (or print screen) showing the purchase transaction (mandatory)
  • Confirmation from retailer on refusal to accept return of purchased item (mandatory)
  • Credit card slip (optional)
  • Dissatisfied item - (to be surrendered to EIB brand new, in good working condition in its original packing including its accessories, manuals, etc… when advised by Claims Handler) (mandatory)
To claim for an extension of the original manufacturer's warranty for the covered appliance, items must be purchased entirely using any of the listed Maybankards for up to 24 months, with the total coverage period including the original manufacturer's warranty not exceeding 48 months.
