You may make the payment using your credit or debit card (Master or Visa) by submitting the completed Credit/ Debit Card Authorisation (Autopay) form to info@etiqa.com.my email.

Nomination

Duly completed form and additional documents required:
  • Nomination form
  • Copy of Identity Card or Birth Certificate of nominee
  • Copy of Identity Card or Passport of named trustee is COMPULSORY, if applicable

Change of address/ Contact details

Duly completed form and additional documents required:
  • Request for change form or letter from the policy owner advising on current correspondence address, telephone number and email address.

Change of auto credit details

Duly completed form and additional documents required:

Duplicate Policy (Loss of original policy)

Duly completed form and additional documents required:
  • Statutory declaration form signed in the presence of a Commissioner of Oath (Pesuruhjaya Sumpah)
  • Stamp duty of RM10.00
  • Administration fee of RM31.80 (inclusive GST)

Change Frequency of Premium Payment

Duly completed form and additional documents required:

Reinstatement

Documents required:
  • Health declaration form duly completed and signed by the life insured, policy owner and witness
  • Full remittance of all outstanding premium including reinstatement fee, if any
  • Medical report, if applicable (the expense is to be borne by the policy owner)
