Our Life Services Insurance Claims Department is responsible for ensuring that all genuine and legitimate claims from our policies are honoured and settled accurately, promptly and professionally. 

  • Original policy contract
  • Death Claim Form
  • Doctor's Statement (when necessary)
  • Certified copy of death certificate
  • Certified copy of deceased's IC as evidence of age if proof has not been received before
  • Post Mortem report (where available)
  • Certified copy of proof of relationship (e.g. Marriage certificate/birth certificate and etc.)
  • Certified copy of claimant's IC
  • Certified copy of police report if death is due to accident
  • Consent letter for medical report extraction
  • Certified copy of Burial Certificate (when necessary)
  • Certified ‘Sijil Faraid'/Letter of Administration (if applicable)
  • Other supporting documents (if applicable)
  • Original policy contract (Optional)
  • Total and Permanent Disability Claim form
  • Medical report
  • Certified copy of assured's IC as evidence of age if proof has not been received before
  • Consent letter for medical report extraction
  • Medical report from SOCSO (optional)
  • Letter of job termination from claimant's employer (if employed)
  • Certified copy of medical chits/attendance report (when necessary)

The time limit during which an insurer may contest or challenge the validity of the policy due to fraud, suppression of material facts or misrepresentation in the application.
Yes. The medical report on Insured's current medical condition is necessary to prove continuity of disability. In the event that the covered person ceases to be totally and permanently disabled, the Company shall discontinue any further installment payments or waiver of premium, where applicable.
Yes, it is advisable to pay the premium until the claim has been approved. Excess premium will be refunded to you, if any.
Where a policy owner dies without having made a nomination, the Grant of Probate or Letter of Administration or Distribution Order will be required to distribute the policy moneys accordingly.
