Documents required for different Life and Family claims
- Death Claim Form / Claimant Statement Form
- Statement of Medical Examiner ( for contract duration less than 5 years)
- Certified copy of death certificate
- Certified copy of Deceased's NRIC
- Certified copy of proof of relationship between claimant and participant (e.g. Marriage certificate/birth certificate and etc.)
- Certified copy of claimant's NRIC/Passport
- Certified copy of 'Sijil Faraid'/Letter of Administration/Grant of Probate (if applicable)
- Other supporting documents (if applicable)
- Additional requirements if Death is due to an accident.
- Certified copy of Police report
- Certified copy of driving license for road traffic accident
- Certified copy of Detailed Post Mortem report (where available)
- Certified copy of Toxicology report (where available)
- Certified copy of Police detailed Investigation report (where available)
- Newspaper/ Online News cutting (where available)
- Additional requirements if Death occurred in Overseas/outside of Malaysia.
- Confirmation letter from National Registration Department (JPN)
