Select the type of claim (e.g: Death Claim)

Follow the guidelines provide and upload the necessary documents

Documents required for different Life and Family claims

  • Death Claim Form / Claimant Statement Form
  • Statement of Medical Examiner ( for contract duration less than 5 years)
  • Certified copy of death certificate
  • Certified copy of Deceased's NRIC
  • Certified copy of proof of relationship between claimant and participant (e.g. Marriage certificate/birth certificate and etc.)
  • Certified copy of claimant's NRIC/Passport
  • Certified copy of 'Sijil Faraid'/Letter of Administration/Grant of Probate (if applicable)
  • Other supporting documents (if applicable)
    Additional requirements if Death is due to an accident.
  • Certified copy of Police report
  • Certified copy of driving license for road traffic accident
  • Certified copy of Detailed Post Mortem report (where available)
  • Certified copy of Toxicology report (where available)
  • Certified copy of Police detailed Investigation report (where available)
  • Newspaper/ Online News cutting (where available)
    Additional requirements if Death occurred in Overseas/outside of Malaysia.
  • Confirmation letter from National Registration Department (JPN)
